About the Conference
The Hawai‘i Primary Care Association
Beginning in 2010, HPCA’s commitment to community-based partnerships and innovation made the association a leader in the movement to address social determinants as part of the solution to Hawai‘i’s health care challenges. In the spirit of that history, HPCA is proud to present ho‘ōla - The Hawai‘i Health Care Conference 2017, in partnership with two organizations that share our vision of thriving, healthy communities throughout the state.
HAWAI‘I STATE DEPARTMENT OF HEALTH
The Department of Health works to promote health and reduce the burden of chronic disease for the people of Hawai‘i by empowering the community, influencing social norms and supporting and encouraging healthy lifestyle choices.
HAWAI‘I PUBLIC HEALTH INSTITUTE
The Hawai‘i Public Health Institute believes that building healthier communities requires collaboration on a variety of issues that impact health, including transportation, education, access to healthy food, and economic opportunities.
